4. If there is a difference between the name of the victim/complainant in the FIR and the name on the Caste Certificate, then a self-declaration letter (affidavit) stating that both the names are the same person

5. Xerox copy of the Aadhaar card of the victim/complainant

6. Xerox copy of the passbook of the nationalized bank of the victim/complainant

7. Copy of the medical examination report of the victim/complainant
